Anticipating the Rural Impact of Medicare Value-Based Purchasing

This report discusses value-based purchasing (VBP), a collection of new Medicare payment programs designed to improve clinical quality and patient experience while reducing cost inflation. It discusses the forces leading to the design and implementation of VBP programs; describes programs for prospective payment hospitals, critical access hospitals, skilled nursing facilities, home health agencies, and physician office practices; and describes the legislative statute, regulations or demonstration project details, rural inclusion, and preliminary results of Medicare VBP programs. This findings brief examines the impact that states’ budget cuts and increased funding by the National Health Service Corps (NHSC) Loan Repayment Program are having on states’ loan repayment programs from 2007 to 2010. It compares joint state-NHSC and state-only repayment programs; examines state-only and state-NHSC repayment programs with increased, decreased, and stable funding; and presents survey results from directors of selected state programs on yearly changes in size and composition of the workforce.
