Abstract: This policy brief reviews factors that may increase or decrease potentially preventable readmissions (PPRs). It explores the relationship between PPRs and use of outpatient follow-up care, discharge destination, rural versus urban residence of the patient, and time to follow-up care in Medicare patients hospitalized with congestive heart failure and bacterial pneumonia in 2006–2007.
