|Abstract:||This policy brief provides a snapshot of patient-centered medical home (PCMH) services and processes delivered by rural primary care practices. It uses a survey instrument developed by the RUPRI Center for Rural Health Policy Analysis to examine PCMH readiness in five domains: access to care, care management, population-based care, quality, and clinical information management.
